1. Mass Athlete Performance (M.A.P.) Waiver

In addition to registration through NSC, all participants are required to complete a participation waiver via M.A.P. This requires an additional registration, but at no extra cost.

2. Board of Health Compliance

This camp must comply with regulations of the Massachusetts Department of Public Health and be licensed by the local board of health

NSC are required to maintain a health record for each camper to include of:

1. Complete of NSC Parent/Camper Packet – This can now be completed electronically via the M.A.P. website in a similar way to the M.A.P. waiver above:

2. Submit A Report of a Physical Examination carried out within 18 months of the clinic start date

3. Submit an up to date Certificate of Immunization
